Patient Look Up

In addition to demographic information, Patient Policy Number can be used as search criteria.  If a patient has more than one visit with the same policy number it will return a row for each visit with each DOS associated with the policy number.  It searches on Patient Policy Number for all insurances, either primary, secondary, or other.
